Consent form and privacy policy
I hereby give my voluntary consent for my employer (data controller) to process and store personal data about me. It will be the consultancy company Health Group (data processor) that will in practice register, process and store personal data about you on behalf of your employer.
In connection with. well-being surveys, workplace assessments and other analytical work, the personal data registered will be
- General information, cf. Article 6 of the General Data Protection Regulation: Name, email address, department if applicable.
- Special categories of personal data, cf. Article 9 of the GDPR: Responses from questionnaire data on physical and mental work environment, health, sickness absence and health information.
The purpose of transferring your personal data to Health Group is to prepare statutory occupational health and safety reports and other reporting to your employer in anonymized form.
You have the right to withdraw your consent at any time. You can do this by contacting Health Group by phone +45 70201626 or email info@healthgroup.dk. If you choose to withdraw your consent, it does not affect the lawfulness of the processing of your personal data based on your previously given consent up to the time of withdrawal. Therefore, if you withdraw your consent, it will only take effect from that point in time.
Your rights
In addition to your right to gain insight into the information that Health Group processes about you, you have the right to access your personal data within the framework of the General Data Protection Regulation:
- the right to have inaccurate information about yourself corrected
- the right to have data about you erased before the time for regular erasure occurs
- the right to have the processing of your personal data restricted
- the right to object to the otherwise lawful processing of your personal data
- the right to receive your personal data in a structured, commonly used and machine-readable format and to have this personal data transmitted from one controller to another without hindrance
Your records are automatically deleted 6 months after the survey is completed, unless we have agreed otherwise with your employer. Your records will not be disclosed to third parties, but will be included at a general and anonymized level in connection with the preparation of a report to your employer. Automated profiling is not performed.

your personal data on the basis of your previously given consent, up to the time of withdrawal. Therefore, if you withdraw your consent, it will only take effect from this time.
Your rights
In addition to the fact that you have the right to gain insight into the information that Health Group processes about you, you have within the framework of GDPR:
- the right to have incorrect information about yourself corrected
- right to have information about you deleted before the time of regular deletion occurs
- right to have the processing of your personal data restricted
- right to object to the otherwise lawful processing of your personal data
- the right to receive your personal data in a structured, commonly used and machine-readable format and to have this personal data transferred from one data controller to another without hindrance
Your registrations will automatically be deleted 6 months after completing the survey unless we have agreed otherwise with your employer. Your registrations will not be handed over to third parties incl. the data controller (employer), but can be included at the overall and anonymous level when compiling statistics, research or trends. No automated profiling is performed.
