Collaboration agreement_ health consultant

Dear PSP consultant

I am pleased to confirm our collaboration agreement between you and Health Group A/S, in which we agree to the following:

Start time

  • You will start as a PSP consultant as of today, reporting to David Sommerlund and Signe Godtkjær Tegner.
  • Your tasks include creating health profiles and related tasks
  • You have a duty to improve your skills within your own field and keep up with the development of the field. You also have a duty to take continuing education, courses, etc. offered by your employer.

Work location and working hours

  • The work is carried out at Health Group's customers.
  • Monthly working hours are variable and depend on needs. You will need to report which days you can work each month and a schedule will be created accordingly.
  • Your working hours will usually be on weekdays. Should night or weekend work occur, you will be informed of this. For night or weekend work, there will be a fee of DKK 100 extra per hour. Working hours are exclusive of a 30-minute daily lunch break. Working hours are agreed with your immediate manager

Salary and pension

  • Salary is individually agreed. There is no pension scheme associated with the position.
  • Settlement is made by submitting an hourly statement through DANLØN APP no later than the 20th of the month.
  • REMEMBER to report salary for each month, postponement or late reporting of salary is not accepted.
  • We pay your salary monthly in arrears, so you'll have it in your bank account on the last working day of the month.

Vacation and holidays

  • You are entitled to holiday pay in accordance with the Holiday Act. You earn 12.5% in holiday pay, which is transferred to your holiday account. Vacation is taken by agreement.

Illnesses

  • In case of illness, you must notify your immediate manager as soon as possible and no later than 7:00 am by phone - not by text message.
  • You must regularly and always upon request keep your immediate manager informed of the expected duration of your sick leave. If you are sick for more than 4 days, a certificate of duration (medical certificate) may be required and you may be called in for a sickness absence interview. The medical certificate is paid for by the employer.
  • Health Group is obliged to pay sickness benefits in accordance with the Danish Act on Sickness Benefits after 8 weeks of employment if the working hours within the last 8 weeks are at least 74 hours.
  • If you receive salary during illness, you must contribute to Health Group A/S receiving sickness benefit reimbursement from your municipality of residence.

Termination and probationary period

  • The employment relationship may be terminated by the Consultant with 14 days' notice.
  • The employment relationship may be terminated by the Employer with 14 days' notice.
  • The first 3 months of employment are considered a probationary period. During the probationary period, both parties may terminate the employment relationship with 14 days' notice, provided that the notice is given no later than 14 days before the end of the probationary period.
  • Upon termination of employment, you must return all material and effects belonging to Health Group A/S, including keys, access cards, documents, PC, mobile phone, etc.

Copyright and confidentiality

  • All material produced during the performance of your work or at the direction of Health Group A/S is the property of Health Group A/S.
  • You have a duty of confidentiality with regard to all matters relating to Health Group A/S, including Health Group A/S' business operations, collaborative relationships, etc. The duty of confidentiality also applies after the termination of the employment relationship, just as you are otherwise covered by section 3 of the Danish Marketing Practices Act and section 4 of the Danish Trade Secrets Act.
  • The Consultant is furthermore obliged to follow the general guidelines issued by Health Group A/S at any given time.
  • Breach of confidentiality will result in immediate dismissal and legal action.

Confidentiality

  • You are required to treat sensitive personal data with confidentiality at all times. The following rules apply to both internal and external workplaces in Health Group A/S:
    • Always log off the computer when leaving the workplace
    • Do not leave staff-sensitive data behind when leaving the room, including in the trash
    • Do not store or remember passwords on the computer
  • Violation of the confidentiality clause will result in immediate dismissal and legal action.

Occupation

  • It is agreed that in connection with your employment in Health Group A/S, you may not, without the consent of Health Group A/S, have a secondary occupation or engage in activities that may be considered incompatible with the position in Health Group A/S.

GDPR

  • The General Data Protection Regulation (GDPR) requires Health Group to secure customers' personal data by having appropriate organizational and technical measures in place. These measures must protect the customer against three things:
    1. Unauthorized use of the company's personal data. Personal data is not for unauthorized persons.
    2. Unlawful processing of the company's personal data.
    3. Accidental loss, destruction or damage of the company's personal data.

    As a Health Group employee you may ONLY access customers' personal data that you have a direct connection to. This means, among other things, that you may not search for an employee within the portal or open a data set even if it is in a customer folder, without a legitimate purpose.

    By signing this form, you confirm that you will only access customers' personal data for a legitimate purpose.

Other provisions

The employment relationship is not covered by collective agreements.

The consultant is obliged to comply with the set of values promoted by Health Group A/S; i.e. that the consultant is a good role model and leads a healthy lifestyle and radiates health and energy. The consultant is furthermore obliged to dress neatly and decently in connection with the performance of health profiles.

You have a duty to exercise due care when using company vehicles or vehicles in the company's custody. The company reserves the right to raise a claim against you if the company incurs a loss, for example as a result of the employee's violation of general traffic rules, including in cases of insane driving or as a result of the employee's use of yellow plate cars in violation of the rules. Special attention is drawn to the fact that you must pay any speeding or parking fines or other types of fines imposed by the company as a result of your violation of traffic laws. This also applies in cases where you have left the vehicle in the care of others. Please also note that violations may have consequences for the employment relationship - depending on the circumstances in the form of dismissal.

You are at all times obliged to keep Health Group A/S informed of your residential address and other matters of importance to your employment with Health Group A/S.

Social media is a big part of our everyday lives, and it's a great place to promote yourself and what you do. Health Group is of course also present on social media (Instagram, LinkedIn and Facebook). We would like to encourage you to follow and like us and to include us in your posts related to working for us. We would greatly appreciate it. Health Group reserves the right to repost your posts so that they also appear on our site. Of course, we do this in a respectful way by tagging you in the post.
